Assists YVFWC clients in obtaining the social and health services they need in an integrated and coordinated manner.

Education: Bachelor's Degree in a social service field such as Social Work, Behavioral Sciences, Psychology or Child Development. 4 years of related experience may substitute for education for positions outside of CHS.
Experience: Two years of experience working in community social services, public health services, crisis intervention, outreach or linkage programs.
Licenses/Certificates/Registration: Current Washington or Oregon driver's license and proof of automobile liability insurance coverage.
Knowledge/Skills/Abilities Required: Basic bilingual proficiency in verbal and written communication preferred1. Must have demonstrated ability to communicate at level 10 on the ALTA language proficiency scale in order to receive bilingual differential. Effective verbal, written and listening communication skills. Strong customer relations skills including the ability to manage stressful situations. Knowledge of medical terminology required. Knowledge of cultural differences and impact on appropriate care provided to patients. Knowledge of local social and health services. Knowledge of how to access local community agencies and resources. Ability to develop effective working relationships with social service providers and partners. Ability to effectively interact with a variety of clients and employees in a professional manner. Basic proficiency with a variety of computer programs including, Microsoft Outlook, Word and Excel.
