Description: The primary function of the Life Coach is to assist each consumer in creating an individual rehabilitation plan. The Life Coach will also help the consumer/family to identify the expected outcomes and ensure that services identified will lead toward positive achievement of consumer chosen outcomes.
Location: Monmouth County
Responsibilities:
- Engages the consumer and his/her family in a proactive manner to learn how each person wants to live.
- Provides on-site services and support.
- Develops an individual rehabilitation plan designed to address needs and strengths through specific services and outcomes within a unique time frame.
- Documents observations, information and services provided to the consumer in progress notes.
- Communicates observations and information about the consumer to the Team Leader.
- Provides transportation, counseling and recreational/socialization support.
- Provides crisis intervention services.
- Provides assistance in facility management.
- Ensures that all reports are filed in a timely and accurate manner.
- Assists with completion of all Quality Assurance plans.
Qualifications:
- A Bachelor’s Degree in a helping profession discipline is required
- Valid NJ Driver’s License with a clean driving record is required
- Working knowledge of and ability to work on computers is mandatory
Salary: 36,000/yr.
