Senior Integrated Care Specialist (Job 10112)

POSITION OBJECTIVE

The Senior Integrated Care Specialist serves as the primary point of contact in leading and facilitating the coordination of care for students served by University Health and Counseling Services (UHCS). The Senior Integrated Care Specialist will be responsible for overseeing the development and implementation of Care Management at UHCS. This role supports the UHCS mission of enhancing the health and well-being of the student community and providing high quality, multidisciplinary mental health-related services. All members of the UHCS staff are considered essential and are expected to work on-campus in accordance with Case Western Reserve University guidance.

ESSENTIAL FUNCITONS

  1. Serve as the main point of contact for all Care Management functions within UHCS. Ongoing leadership of the development and implementation of Care Management processes, as well as oversight of scheduling and duties of the UHCS Care Management Team. (25%)
  2. Lead and facilitate the UHCS Care Management Program, including: provide care management and referral coordination to students with complex health needs; ensure connection to UHCS, university, and community provider resources; monitor and track medically ill, high risk and crisis management students via the electronic database. Frequent follow-up and monitoring of these students to assess treatment plan adherence and to encourage engagement in care; engage with students upon discharge from the emergency room, inpatient or intensive outpatient hospital care to facilitate treatment within UHCS and/or with community providers; document and follow up with all students who use UHCS on‐call counselors; attend Care Management meetings, in collaboration with the UHCS Care and Crisis Management team, the Dean of Students Office, and Student Advancement (Navigators); escalate care management issues, as necessary, to the clinical Integrated Leadership Team; consult with parents, faculty, school administrators, student affairs staff, when appropriate, to ensure quality care to students (timeliness, access, follow up); and maintain appropriate records and documentation of care management activities, in compliance with law, ethics, and best practice. (30%)
  3. Develop and maintain collaborative partnerships and communication with campus resources and community resources related to the provision of Care Management, including maintaining an up-to-date referral database for community providers and basic needs resources; maintain a basic knowledge of the Student Medical Plan, for referral purposes. (10%)
  4. Provide direct counseling and mental health services to students presenting with a range of mental health and wellness needs within the established counseling model. Services include same-day initial contact sessions, same-day crisis management sessions, workshops, and ongoing individual and group counseling sessions. (20%)
  5. Oversee Care Management participation in all campus Behavior Intervention Team (BIT) meetings, including Undergraduate and Graduate BIT Meetings, as well as the BIT meeting at the Cleveland Institute of Art. Participate in Student Affairs or University committees and activities, e.g., student orientation professional development, or faculty-staff committees as assigned by the Associate Director of Counseling. (10%)

NONESSENTIAL FUNCTIONS

  1. Design and contribute material for UHCS to help incoming students transition successfully into the university. (2.5%)
  2. Perform other duties as assigned. (2.5%)

CONTACTS

Department: Daily contact with professional and support staff of University Health and Counseling Services (UHCS). Continuous contact with Executive Director of UHCS, Director of Health Services, Director of Clinical Operations, the UHCS leadership team, clinical nurse manager and departmental staff to work collaboratively.

University: Frequent contact with other offices within the Division of Student Affairs. Frequent consultation services for faculty, staff and administrators seeking to help students. Periodic liaison contacts with deans, faculty and/or administrators involved in the various schools within the university.

External: Frequent consultation with regional hospital emergency, medical and psychiatric care professionals in order to develop patient discharge plans (i.e., post-hospital care) that may include community and/or university mental health treatment options. Collaboration with health professionals in the community to assist in the referral of students to appropriate services, including: specialty care, individual or group therapy, intensive outpatient services, and in-patient hospital treatment. Occasional contact with community groups regarding health issues and in the promotion of Case Western Reserve University. Occasional contact with regional and national peers to present papers, projects, ideas and research.

Students: Daily contact with students seeking care, including: care management, risk assessments, individual counseling, group counseling, crisis intervention, and referral coordination and bridge sessions. Educational services to student groups and organizations, especially students in Residence Life and Services and Greek Life and/or students in the professional schools and institutes, i.e., Cleveland Institute of Music, Cleveland Institute of Art, Kent State University College of Podiatric Medicine, etc.

SUPERVISORY RESPONSIBILITY

In collaboration with the Associate Director of Counseling, provide administrative supervision of Care Management Team.

QUALIFICATIONS

Experience: Minimum 4 years of experience in mental/medical health care. Prefer experience working at a Higher Education Institution or internship experience in Higher Education Case Management.

Education/Licensing: Master’s degree in Counseling with independent license/license-eligible in the state of Ohio (LPCC or LPCC-S) or Master’s degree in Social Work with independent license/license-eligible in the state of Ohio (LISW or LISW-S). LPCC-S or LISW-S preferred.

  5. Demonstrated knowledge working with an electronic medical and psychological record system, especially as it relates to timely reports and the protection of privacy.
  6. Ability to adhere to federal and state laws and university protocols designed to protect the privacy rights of students.
  7. Ability to meet consistent attendance.
  8. Ability to interact with colleagues, supervisors, and customers face to face.

WORKING CONDITIONS

Standard Health and Counseling offices in a university campus setting.

Full-time professionals are exempt from overtime and are expected to work a minimum of 40 hours per week around the university’s core hours of operation. UHCS outreach and campus emergency response efforts require UHCS staff to periodically work evenings and weekends.

Staff may be required to adjust their clinical and administrative assignments, as needed, depending on departmental, divisional and university needs. This may involve flexing to cover other clinical roles (e.g., covering acute counseling care responsibilities, covering ongoing counseling responsibilities, alternating between roles, etc.) due to staff absence, changes in the model of care, or other reasons determined by the Executive Director of UHCS or their designee.

Staff are expected to provide services on-site and in-person, with the understanding that there may be aspects of their role and responsibilities that could shift to virtual and/or via hybrid modalities, depending on university requirement, departmental need, public health guidance and/or at the discretion of the Executive Director of UHCS or their designee.
